Note: In order to make your visit to our office as smooth as possible, our schedulers will obtain information from you to avoid problems at the time of the appointment. This will include information such as Name, Date of Birth, Social Security Number, Phone Number, Address, Referring Physician, Insurance Information, Type of Examination, and Reason for the Examination.
The appointment scheduler will give you an appointment time and date. Certain examinations require a preparation which will be given at this time as well.
We have the capability to speak with our patients while utilizing an interpreter, whether we are face-to-face or on the phone. The following are just a handful of the 119 languages our interpreters can speak: Mandarin Cantonese Russian Yiddish
If you have any questions regarding pre-authorization requirements, please feel free to ask this at the time of scheduling. If you have any questions or concerns about your coverage that we cannot provide adequate answers for, please call your carrier for details.
